Restore Oncology Care and Lymphoedema Clinic (“Restore”, “we”, “us”, “our”) is committed to protecting the privacy of our clients’ personal and health information. This policy explains what information we collect, how we use and store it, and your rights in relation to it. 

As a health service provider, we handle your information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), and the NSW Health Records and Information Privacy Act 2002 (HRIP Act) and its Health Privacy Principles, which apply to health information collected in New South Wales. 

Information we collect 

We collect personal and health information necessary to provide safe, effective care, including: 

  • Contact details (name, address, phone, email, date of birth, emergency contact) 
  • Medicare, DVA, NDIS, private health insurance and referral details 
  • Health information relevant to your treatment, including medical history, clinical notes, assessment findings, correspondence with your referring doctor or other treating practitioners, and treatment records 
  • Payment and billing information 

How we collect it 

Directly from you (in person, by phone, email, or through our online booking system), from your referring practitioner, or, with your consent, from other treating health professionals involved in your care. 

Why we collect and use it 

To provide and coordinate your clinical care, to communicate with you about appointments and your treatment, to process payments and insurance/Medicare/NDIS claims, to meet our legal and professional obligations (including AHPRA record-keeping requirements), and to manage and improve our services. 

Who we may share it with 

Only as necessary for your care or as required by law, including: your referring doctor and other treating health practitioners involved in your care, Medicare, DVA, NDIS or your private health insurer (for claims and funding purposes), our booking and practice management software providers (currently Splose) and payment processors (currently HICAPS), and regulatory or legal bodies where required by law. We do not sell your personal information, and we do not disclose it to third parties for marketing purposes without your consent. 

Storage and security 

Your information is stored securely in our practice management system and is accessible only to authorised staff involved in your care. We take reasonable steps to protect your information from misuse, loss, unauthorised access, modification or disclosure. 

Access and correction 

You may request access to your health records, or ask us to correct information we hold about you, at any time. Please contact us using the details below. We may charge a reasonable administrative fee for providing copies of records, in line with applicable regulations. 

Website and online bookings 

Our online booking system may collect limited technical information (such as browser type and IP address) for the purpose of operating the booking service securely. We do not use this information for tracking or advertising purposes beyond what is needed to run our website. 

Data breaches 

In the unlikely event of an eligible data breach involving your personal information, we will act in accordance with the Notifiable Data Breaches scheme under the Privacy Act.

If you have concerns about how we have handled your personal information, please contact us first at info@retsore.physio or 02 75015013 so we can resolve the matter directly. If you are not satisfied with our response, you may lodge a complaint with the NSW Information and Privacy Commission (IPC) or the Office of the Australian Information Commissioner (OAIC).
